MyChart

MyChart allows you  to access and print your child’s immunization records, request appointments online, review your child’s appointment history, and more.

To set up a free and secure MyChart account:

• Download and complete the activation form and turn it in to the front desk staff at your Texas Children's Pedatrics practice (it cannot be accepted by fax or e-mail).

 OR

•  Visit your Texas Children's Pediatrics practice and ask the front desk for an activation form. Complete the form and return it to the front desk staff.

You will receive an e-mail notification within 2 weeks confirming your MyChart access and providing you with your activation code.
